QUESTION: How should the complaint be communicated be communicated?
ANSWER: The complaint must be in writing and signed by the Complainant and an authorised representative, if any, it must state the names and addresses of both the Complainant and the branch/agency of the Bank or insurance company; the facts giving rise to the complaint supported by documents (if any); the nature and extent of loss caused to the Complainant and relief sought from the Ombudsman.
